Map and Directions

Use the map below to get directions to our Clearwater, FL dentist office.

Please contact Joanne to schedule an initial exam or radiographs. We do accept emergency patients if you are experiencing acute discomfort or have serious concerns.

Because we are grateful you have chosen us out of the many available Clearwater, FL dentists, we try to make requesting an appointment as easy and efficient as possible. You can request a date and time through our website, and our scheduling coordinator will contact you to confirm.

If you are delayed or unable to appear for a scheduled appointment, please let us know as soon as possible.

Although we are a non-participating health care provider, we accept most PPO dental insurance plans and are happy to file the claims for you, as long as you are free to choose your own dentist.

Unfortunately, we are unable to accept HMO plans. Our accepted payment methods are as follows: checks, cash, or credit cards. We also offer customers a flexible payment plan to make dental services more affordable.

Please discuss these matters with our Financial Coordinator.

Our easy financing option, CareCredit, helps you afford treatments or procedures that your health care coverage does not pay for. Our No Interest* financing and low minimum monthly payment options make it easy to afford the dental care you want or need immediately.  CareCredit even applies to your family and pets.

CareCredit requires only three easy steps and offers instant approval, so the application process is a breeze and you don't need to wait long to learn if you qualify. Once approved, you can use CareCredit for any type of medical care, including LASIK surgery, veterinary services, dentistry, cosmetic repairs, and even hearing aids.

CareCredit has endorsement from many credible healthcare organizations, it is a GE Money Company, so you know it is reputable. The service has helped over five million cardholders get the health care they need over the last 20 years.

Our office has recently upgraded to digital radiography, which uses less radiation than traditional imaging systems do.

Our system features an intraoral camera and monitors in each treatment room, which enables Dr. Slaybaugh to educate patients about their oral health in an innovative, visual manner.

In addition, since our building was—from the design stage forward—intended to be a dental office, each room is large, comfortable, and soothing, and features a relaxing view of the outdoors.

Cash, Visa, MasterCard, American Express, and Care Credit are the payment options we accept. As mentioned above, we also offer a flexible payment plan for your convenience. For more information, please ask to talk to our Financial Coordinator.
